Joining the Dots: a Meeting of Minds

This report encapsulates a two-and-a-half-hour meeting convened by Together for Change in partnership with public, third sector, and community organisations. Rooted in the Ten-Point Plan for Community-led Action and informed by experiences during the pandemic, the meeting explored avenues for reinforcing communities in the face of challenges.

The presentations delved into the implementation of The Well Being of Future Generations Act, tools for understanding community needs, and the impact of collaborative efforts on proactive and planned care. Group discussions delved into challenges, priorities, and the imperative to work with communities on social determinants.
